Care Plan / Power of Attorney

For patients who need care plans e.g. those with long-term conditions. Research shows us whether our lives are long or short, recording our wishes for our health and welfare are best done before these conversations become paramount, when we have time to think, plan and record these wishes and then get on with living.

Please complete the Care Plan - Power of Attorney questionnaire below, that would help us to update your record to include wishes about your future care.

The questionnaire includes information about your nominated Next of Kin and /or Power of Attorney, as well as questions about your preferred place of care if you become unwell.

Please complete the information attached and return it to us so that we can begin updating our records with your wishes in mind.  It is important for you to know that this information can always be changed when and if you change your wishes simply by speaking to your GP.  It is also important to know that this information is shared with your GP practice and other health care professionals such as the community nursing team and London Ambulance service so that your care is coordinated and your wishes are known.
